The authors recommend these strategies to help prevent medical mistakes:

* A core group of clinicians--doctors, nurses, and others--should be trained in modern safety principles at every hospital.

* Two patient safety directors--a physician and a nurse or pharmacist--should have "the clout, independence, and resources to measure errors, conduct routine audits of error-prone areas, coordinate detailed investigations."

* Patient safety directors should be equal to, not under, other hospital departments. And they need a budget.

* Safety must be everyone's job. When the core team ...
