1. NO PURCHASE NECESSARY. 2. How to enter: Fill out the questionnaire, and print your name, full address, phone number, and email address. Entries may be submitted by mail or Internet. By mail: Send to Allure, 7 Tecumseh Court, Commack, NY 11725. By Internet: Log on to allure.com. All entries must be received by 11:59 p.m. EDT on June 8, 2004. Entries that are lost, late, misdirected, garbled, have postage due, or are not fully received for any reason -- including by reason of hardware, software, browser or network failure, malfunction, congestion, or incompatibility at sponsor or elsewhere -- will not be eligible. Survey responses without name, address, and email address (for Internet entries) will not be eligible or counted. Only one entry per person/email address permitted. If you do not want your email address used for future marketing purposes, please check the appropriate box on the entry form. 3. One Grand Prize, one Big Beauty Makeover Prize, and one Big Beauty Relax Prize winner will be chosen in a random drawing from all entries on or about July 7, 2004, by Accu-Tab Computing and will be notified by phone or mail within two weeks of the drawing.
