We are in the era of antibiotic-resistant Propionibacterium acnes, and that calls for combination therapy, according to speakers at the annual Hawaii dermatology seminar sponsored by the Skin Disease Education Foundation.
That means benzoyl peroxide is coming back into vogue as first-line treatment, said Dr. Lawrence F. Eichenfield of the University of California, San Diego. Benzoyl peroxide has not been shown to induce resistance in P. acnes, and used in combination with an antibiotic, it can cut down populations of antibiotic-resistant bacteria.
Without a doubt, resistance does have clinical significance, he said. Resistance has now been shown to all the commonly prescribed oral antibiotics and most of the antibiotics used topically.
