Kudos to Dr. David M. Priver (no relation) on his comments about episiotomies, as well as forceps ("Movement Taking Step Back," Letters, Feb. 1, 2004, p. 7).
I, too, have done thousands of episiotomies over nearly 30 years with no rectovaginal fistulas or fecal incontinence problems other than immediate postpartum. I have, however, seen relaxed vaginal tissues, including cystoceles and rectoceles, associated with the prolonged pushing of natural childbirth without medications.
