The most time-efficient work-up of headache consists of four key questions and a brief search for "red flags," two speakers said at the annual meeting of the American Headache Society.
Organic causes account for 1%-2% of headaches seen in primary care offices. Don't forget about these "secondary" headaches, but think first of migraine, advised Dr. Merle L. Diamond of the Diamond Headache Clinic, Chicago. "That's your default diagnosis."
Dr. Diamond defines migraine as a recurrent disabling headache. Four questions should provide enough information to diagnose migraine or to seek another cause:
* Do you have a recurrent disabling headache? (Most are migraines.)
* Do you have a family history of headache? (If so, migraine is more likely)
* Do you take medicine for headaches more than 2 days a week? (Frequent use of over-the-counter analgesic medications may cause rebound headaches.)
