LAKE BUENA VISTA FLA. -- Suspect levator ani defects in parous patients with unexplained pelvic pain, Dr. Martin J. Quinn advised.
Of 26 parous women with unexplained pelvic pain, 20 (77%) demonstrated levator ani defects when subjected to T2-weighted, fast spin-echo and MRI in the axial and sagittal planes, he reported at the annual meeting of the Society of Gynecologic Surgeons.
In the axial sections at the level of the vesical neck and perineal membrane, several patterns of levator ani defects were observed, both unilaterally and bilaterally. These included avulsion of the pubococcygeus from its insertion point in 3 patients, avulsion of the pubococcygeus from its origin in 14 patients, and loss of conformation and muscle bulk consistent with denervation of the pubococcygeus in 8 patients.
Several patients had more than one defect, said Dr. Quinn of Hinchingbrooke Hospital in Huntington, England.
