DALLAS -- Total colpocleisis combined with site-specific reconstructive surgery is a useful, effective treatment for elderly women with severe uterovaginal prolapse who have no further interest in sexual intercourse, Dr. Ozgur H. Harmanli said at the annual meeting of the Society of Gynecologic Surgeons.
In the largest case series to date in the English language literature, 41 consecutive women underwent colpocleisis at Temple University Hospital, Philadelphia, between November 1994 and June 2001.
The patients ranged in age from 68 to 90 years, with an average age of 76.4 years. Approximately 70% had previously undergone hysterectomy 20% had had prior incontinence surgery, and 36% had already undergone at least one procedure for prolapse, said Dr. Harmanli of Temple University, Philadelphia.
The implications of the procedure regarding future sexual activity were discussed before surgery was undertaken, and all women initially underwent an objective assessment of site-specific defects. "Then, vaginal denudation was performed in four quadrants, and the raw surfaces of the vaginal area were sutured in a row-by-row, front-to-back fashion," he said.
