Rules Deter Cholesterol Screening
I would like to respond to the article "Cholesterol Tests Slip Past Ob. Gyns." (Oct. 15, 2000, p. 1).
How can we put enough effort into screening our patients for cardiovascular disease when managed care guidelines prohibit our doing laboratory work in our offices? Instead, we must give the patient a referral to a laboratory of the HMO's choosing, thereby inconveniencing our patients by forcing them to make an additional visit for their health care.
I have been trying to screen my patients for 40 years by checking their chemistries, hematocrits, etc. I cannot even do that now in my office as the HMO will not authorize or pay for it. Based on the history I take, I am fully capable of writing a script for Lipitor. My hands are tied. Pity the patients.
