WALNUT CREEK, Calif. _ Reflecting predictions that employers and workers will increasingly shoulder the burden of rising health care costs, PacifiCare of California announced Thursday new plans that lower premiums but substantially raise co-payments for doctor visits and procedures performed outside a few select hospitals.

The plans give employers options and a way to offset premium costs that have risen markedly over the past few years and will once again increase by more than 10 percent in 2002. Employers choosing the plans will save five to 20 percent.
