SAN DIEGO -- Women with endometriosis more commonly have systemic lupus erythematosus, multiple sclerosis, rheumatoid arthritis, Sjogren's syndrome, fibromyalgia, chronic fatigue syndrome, hypothyroidism, allergy, and asthma, compared with women in the general U.S. population, according to results of a survey of 3,680 women.
The increased prevalence of autoimmune disease in women with endometriosis supports the theory that altered immune surveillance plays a role in the etiology of endometriosis. Physicians therefore should consider the possibility of coexisting endocrine and autoimmune disease in women with endometriosis, Ninet Sinaii said at the World Congress on Endometriosis.
In addition, 70% of respondents first experienced pelvic symptoms of endometriosis before the age of 20, yet 74% were diagnosed with the disease between the ages of 20 and 35 years. That finding underscores the importance of considering a diagnosis of endometriosis in adolescents who present with pelvic pain, urged Ms. Sinaii of the National Institute of Child Health and Human Development, Bethesda, Md.
She and her associates analyzed data from a 1998 survey of 4,000 members of the Endometriosis Association, focusing only on the 3,680 respondents who said they had been surgically diagnosed with the disease. The investigators calculated prevalence rates based on survey responses and compared those responses to disease rates for the general population of U.S. women compiled from the medical literature, the U.S. Census Bureau, the Centers for Disease Control and Prevention, and other sources.
Among the women with endometriosis, 42% reported endocrine disease, 31% said they had fibromyalgia or chronic fatigue syndrome, and 12% reported an autoimmune inflammatory disease. Some women reported diagnoses from more than one of these categories: 9%, for example, said they had either fibromyalgia or chronic fatigue syndrome plus an endocrine disease.
