    Everyone makes mistakes. But in the zoo world, a mistake _ a momentary lapse of concentration _ can be lethal. 
    And that appears to be what caused the recent death of a keeper at the Miami Metrozoo. What hasn't died with him are old-fashioned notions about how and why zoo tigers kill. 
    David Marshall, an experienced keeper, was killed by a 350-pound white Bengal tiger. This was probably the third fatality caused by tigers in U.S. zoos in 10 years. The others were at the Bronx Zoo/Wildlife Conservation Park in 1985 and at the Houston Zoo in 1988. 
    In all three cases, it appears keepers overlooked or ignored safety instructions. In New York, as in Miami, a keeper entered the tiger area while the tigers were in it, clearly a violation of zoo policy; in the Houston case, a…
