BERLIN -- Women with systemic lupus erythematosus have a greater prevalence of cervical abnormalities detected on Pap smears than do healthy women; they are at heightened risk for infection with high-risk types of human papillomavirus and for the development of high-grade squamous intraepithelial lesions. Dr. Lai-Shan Tam said at the annual European Congress of Rheumatology.
In a cross-sectional study that included 85 patients with lupus whose mean age was 42 years and 2,080 healthy controls whose mean age was 44 years, the prevalence of an abnormal Pap smear was 16.5% in patients and 5.7% in controls, a statistically significant difference.
Atypical squamous cells of undetermined significance (ASCUS) were prevalent in 4.7% of lupus patients and 3.7% of controls.
But lupus patients had a significantly higher prevalence of pre-cancerous squamous intraepithelial lesions (SIL). The overall prevalence of SIL was increased 5.9-fold in the lupus group, at 11.8%, compared with 2% in the control group.
Low-grade, early-stage SIL was detected in 8.2% of lupus patients and 1.5% of controls, which was a 5.5-fold increased risk, said Dr. Tam of the department of medicine and therapeutics, Prince of Wales Hospital, Hong Kong.
