FORT LAUDERDALE, FLA. -- Patients with vaginal prolapse who wish to remain sexually active are good candidates for abdominal sacrocolpopexy Dr. G. Willy Davila said at a symposium on pelvic floor disorders sponsored by the Cleveland Clinic Florida.
The procedure is maximally invasive, but the success rate is at least 93%, said Dr. Davila, head of urogynecology and reconstructive pelvic surgery at the Cleveland Clinic Florida in Weston.
In general, the best candidates for the procedure are those who need a normal, functional vagina for sexual activity. Someone younger and very sexually active is an ideal candidate. A patient whose vagina is foreshortened will also benefit from this abdominal route, as will one with a very hypertrophied vagina or one who needs additional abdominal surgery.
"If you perform a lot of reconstructive surgery you need to be comfortable opening up the presacral space" for this procedure, Dr. Davila said.
For those who feel comfortable with it, abdominal sacrocolpopexy can be used in women with varying degrees of vaginal vault prolapse--whether the enterocele is obvious, with a clear view of the vaginal apex, or the prolapse is partial.
"Women who have had the procedure look perfectly normal," Dr. Davila noted.
