ST. LOUIS -- Vaginal swabs self-collected by adolescents appear to be a reasonable alternative to clinician-collected cervicovaginal swabs when testing for human papillomavirus, Dr. Jessica Kahn reported at the annual meeting of the Society for Adolescent Medicine.
Because the results of self-collected and clinician-collected samples didn't differ significantly, self-collection might increase screening among adolescents who are averse to speculum exams or who are seen in nontraditional settings, said Dr. Kahn of Children's Hospital Medical Center, Cincinnati.
It has been shown that self-collected and clinician-collected samples in adults yield similar human papillomavirus (HPV) diagnostic results. But adolescent self-collected samples have never been compared with clinician-collected samples, Dr. Kahn said.
"Because adolescents may differ from adults in their ability or confidence to self-collect samples, it was unknown whether the results would be similar or whether self-collection is feasible" in this population, she said.
She examined agreement in HPV identification in 99 paired samples collected from 101 females aged 14-21 years; two of the samples did not amplify. Each of the patients self-collected a vaginal swab. Then, a clinician collected a cervicovaginal swab. Both swabs were tested for HPV DNA with polymerase chain reaction amplification; the cervicovaginal swabs were also examined for cytology.
The patients were seen at an urban teen health center. Their mean age was 17 years; 84% were black. Their mean number of lifetime sexual partners was six. Forty percent had a history of chlamydia, and 27% had a history of abnormal cervical cytology.
