CHICAGO -- Recent antibiotic use, a recent urinary tract infection, and treatment of less than 10 days' duration significantly increase the risk for treatment failure in community-acquired pyelonephritis, Anita G. Carrie said at the annual Interscience Conference on Antimicrobial Agents and Chemotherapy sponsored by the American Society for Microbiology.
Increasing resistance among uropathogens to trimethoprimsulfamethoxazole (TMP-SMX) is believed to decrease the effectiveness of this agent relative to the fluoroquinolones in treating community-acquired pyelonephritis. In a recent Canadian study 19.2% of outpatient uropathogens were resistant to TMP-SMX, compared with just 1.8% resistance to ciprofloxacin (Antimicrob. Agents Chemother. 44[4]:1089-92, 2000).
However, such data may not reflect what happens in routine clinical practice, noted Ms. Carrie of the University of Manitoba, Winnipeg.
