The state botched a project to eliminate suicide hazards at the Winnebago Mental Health Institute and left the building open to further attempts.
It was an oversight, said David Helbach, secretary of the Department of Administration's Division of State Facilities. We didn't do enough preplanning.
He said the state misunderstood its directions on a project to upgrade the facility, and it focused repairs only on living quarters instead of on every area in the buildings.
