SCOTTSDALE, ARIZ. -- Physicians should become more selective in identifying patients who are appropriate candidates for uterine artery embolization for the treatment of fibroids, Dr. Francis L. Hutchins Jr. advised.
Since uterine artery embolization (UAE) was first used to treat fibroids in 1995, simplistic indications for treatment have guided patient selection: Any woman with symptomatic fibroids who wanted to avoid surgery could be considered for UAE.
Some patients, however, are more desirable candidates than others, Dr. Hutchins said at an international congress on uterine fibroids sponsored by the American Association of Gynecologic Laparoscopists.
Dr. Hutchins of Lake Forest, Ill., said the best candidates for UAE include:
* Patients who need uterine fibroid treatment but who are poor surgical candidates.
* Patients who object to the possibility of a transfusion for religious or other reasons, as might a Jehovah's Witness.
* Patients with acute, intractable hemorrhage from fibroids in whom surgery should be avoided to prevent the need for transfusion and other emergent care.
