SAN FRANCISCO -- Pregnant women with periodontitis who underwent a deep tooth cleaning and root planing procedure had a significantly lower rate of preterm birth than those who received no treatment in a pilot study of 366 women.
Previous studies have shown that periodontitis in pregnancy greatly increases the likelihood of preterm birth. The current study is the first to suggest that "mechanical" deep tooth scaling and root planing using a small instrument to clean teeth above and below the gums helped prevent preterm births in women with the disease.
Adding adjunctive antibiotic therapy with metronidazole (Flagyl) to mechanical treatment, however, did not improve out-comes, Marjorie Jeffcoat, D.M.D., reported at the annual meeting of the Society for Maternal-Fetal Medicine.
The researchers screened women at 20-22 weeks' gestation and randomized those with periodontitis to one of three groups. Patients in a control group had a standard surface cleaning and polishing and took a placebo capsule t.i.d. for 1 week. Patients randomized to mechanical treatment alone received deep scaling, root planing, and polishing. They also took the placebo capsules. The third group underwent mechanical treatment and took Flagyl 250 mg t.i.d. for 1 week.
Women who received mechanical treatment alone had the lowest rates of delivery before 37 weeks' gestation (4%) and 35 weeks' gestation (1%).
In a separate reference group of 723 pregnant women with periodontitis recruited from the same population but not treated, 14% delivered before 37 weeks of gestation, and 6% delivered before 35 weeks, said Dr. Jeffcoat, chair of periodontics at the University of Alabama, Birmingham.
The mechanical treatment was well accepted by patients. "Many more women wanted treatment than could be enrolled in this pilot trial, Dr. Jeffcoat said.
