On a morning in November 2004, Theo van Gogh, a documentary film-maker, left his home in Amsterdam on a bicycle but was waylaid by a Moroccan man with a handgun and two butcher's knives. The assailant shot the film-maker, sawed into his throat with one knife and with the other stabbed a five-page letter onto his chest. "Can't we talk about this?" the victim cried as he was attacked.
