PARIS -- Intrauterine hematoma during the first trimester carries a high risk of adverse pregnancy outcome, Dr. Gianpaolo Maso said at the 13th World Congress on Ultrasound in Obstetrics and Gynecology.
In pregnancies affected by intrauterine hematoma (IUH), gestational age at the time of diagnosis is a significant predictor of outcome, whereas hematoma volume and maternal age do not appear to be so, said Dr. Maso of the Instituto per L'Infanzia, Trieste, Italy.
In a retrospective review of 182 cases of IUH diagnosed over a 7-year period by transvaginal ultrasound in the first trimester of a viable pregnancy, clinical complications occurred in 49% of cases. The median age of the women with IUH was 30.8 years, the mean IUH volume at diagnosis was 4.9 mL, and the mean gestational age at diagnosis was 8.2 weeks.
Spontaneous abortion was the most common of the 89 adverse outcomes, occurring in 14% of the IUH pregnancies. Premature rupture of membranes occurred in 13%, and premature delivery occurred in 7%.
