The Patient
A 48-year-old woman presents to the urgent care center with 6 days of facial pain, nasal drainage, and sinus congestion. She denies antecedent respiratory symptoms. This is a teaching situation and the first-year resident looks up the recommended treatment. The source states that antibiotics are the mainstay of treatment for acute bacterial sinusitis, but some faculty members no longer routinely prescribe antibiotics to such patients. The patient says she comes in when she gets these symptoms and always gets an antibiotic. When she has failed to get antibiotic therapy in the past, her symptoms got worse. How should we manage this patient?
