The American College of Obstetricians and Gynecologists is making a huge mistake in trying to modify the intervals at which women should have Pap smears, no matter how sound the reasoning ("ACOG: Annual Pap Smear Not for All Women," Sept. 1, 2003, p. 1).
Even in my upscale suburban private practice, I have many patients who take 2 years or longer to show up for their annual Pap. I have found carcinoma in situ more than once in patients who had a normal Pap smear 2-3 years earlier. The noncompliance that will probably occur in less educated patient populations will be disastrous. I especially cannot believe that ACOG is telling sexually active teenagers that they can have a Pap smear every 3 years (based on risk factors).
The problem is that most patients do not make a distinction ...
