Vaginal tape treats stress incontinence in obese. (Less Blood Loss than with Sling Procedure).
NEW ORLEANS -- Tension-free vaginal tape may be the treatment of choice for genuine stress urinary incontinence in obese women, Dr. Maurice K. Chung said at an international congress sponsored by the Society of Laparoendoscopic Surgeons.
In a study of 156 women with stress urinary incontinence, 92 women were treated with the tape, marketed by Gynecare as TVT Tension-Free Support for Incontinence, and 64 women were treated with a pubovaginal sling. A total of 70% of the TVT group and 49% of the sling group were obese. Both procedures were found to be safe and effective, said Dr. Chung, a consultant for Gynecare who received a grant from the company to conduct the study.
During the 24-month follow-up, cough stress testing revealed that 89 of the TVT patients were dry and 3 reported improvement. In the sling group 14 were dry, 41 reported improvement, and 9 failed therapy, said Dr. Chung of the Medical College of Toledo (Ohio) and codirector of the Midwest Regional Center for Chronic Pelvic Pain and Bladder Control in Lima, Ohio.
Operating time for TVT averaged 25 minutes, compared with 95 for the sling procedure. The length of stay for the TVT procedure was less than 1 day compared with 2-3 days for the sling procedure.
Blood loss averaged 100 cc in the TVT group, compared with 220 cc in the sling group. Half of the patients in the TVT group and all those in the sling group were discharged with a Foley catheter.
The excellent outcomes in the TVT group, which had a higher overall body mass index than the sling group, suggest that the procedure, when performed by a surgeon experienced in the use of TVT, can be considered the appropriate and preferred treatment for stress incontinence in obese women, Dr. Chung said.
No bladder, bowel, or vascular injuries occurred during procedures in either of the groups. American Urological Association scores, which pertain to the degree of irritable voiding symptoms, decreased by about 50% in both groups, and voiding was normal in 90% of those in the TVT group and 70% of those in the sling group at postoperative day 7.
