SAN DIEGO -- The ideal staff of a pain clinic should include Oprah, Marcus Welby, and a really stern drill sergeant, according to speakers at a meeting sponsored by the International Pelvic Pain Society.
A passion for helping difficult patients is essential for anyone who is thinking of starting a pain clinic because "this is a very needy patient population." said Dr. C. Paul Perry of Brookwood Women's Medical Center in Birmingham, Ala.
But precisely because the patients can be so needy, the doctor must know when to practice tough love and draw boundaries. Dr. Perry favors written contracts with patients regarding the use of opioids. Patients must agree to comply with the prescription as written and with the other aspects of their treatment plan. Patients must return to the clinic when their prescription needs renewal, a task that requires some planning ahead, since it can take 2-4 months to get an appointment. Should they lose the prescription or run out of medication before then, they must wait until their appointment to get more.
Superb time-management skills are also important. Practitioners at pain centers should be available for their patients but at the same time set limits to prevent burnout. Dr. Perry said he sees only three new patients a day because each new-patient visit lasts 60-90 minutes.
Because pain is such a complex condition, a pain clinic should have access to specialists in psychology or psychiatry neurology, anesthesiology, physical therapy, pharmacology physiatry and addiction medicine, Dr. Perry said.
