Prenatal diagnosis of monoamniotic twin pregnancy followed by regular fetal surveillance and appropriate administration of corticosteroids can reduce perinatal mortality from previous estimates of 30%-70% to approximately 10%, according to the largest case series reported thus far.
A retrospective chart review of all twin pregnancies at the University of Toronto Perinatal Complex by Dr. Victoria M. Allen and her associates identified 25 women with monochorionic, monoamniotic twin pregnancies that had been detected prenatally.
Of these, serious congenital abnormalities were seen in seven of the twin sets; these included acardia and anencephaly. Intrauterine or neonatal death occurred in five of the cases; in two of these cases, one twin survived. In one surviving twin, the cord encircled the neck four times, and in another twin, the umbilical cord encircled the neck six times.
