The US's unexpected announcement in the State of the Union speech of an extra $10bn (#6bn) to combat HIV-AIDS in poor countries over the next five years has drawn praise and puzzlement in equal amounts.
The fund, which will triple current US efforts of about $1bn a year spent fighting AIDS abroad, is the latest in a series of attempts by the US to seize the initiative in the global fight against the disease.
