Radical mastectomy did not improve overall survival, compared with less extensive surgery, among women with breast cancer who were followed for 25 years, reported Dr. Bernard Fisher of the University of Pittsburgh and his colleagues.
For the randomized, controlled trial initiated in 1971, 1,079 women with negative axillary nodes underwent either radical mastectomy, total mastectomy with postoperative irradiation (but no axillary dissection), or total mastectomy with axillary dissection if nodes became positive. An additional 586'women with positive axillary nodes underwent radical mastectomy or total mastectomy with postoperative irradiation.
Of those with negative nodes, the probability of survival was 25% in the radical mastectomy group, 19% in those who underwent total mastectomy plus radiation, and 26% in those who underwent total mastectomy alone.
