Keeping your hands off the perineum during delivery may lower the risk of a severe perineal tear and make an episiotomy less likely, Dr. Klaus Mayerhofer of the University of Vienna, and his associates reported.

In a prospective, multicenter study, 1,076 women with uncomplicated, term pregnancies were randomized to management with either the hands-on or hands-poised approach in the second stage of labor.

In the traditional hands-on method, the healthcare provider places the left hand on the infant's head to apply pressure in an attempt to increase flexion.
