Traditional Cerclage Removal
I am in complete agreement with Dr. Frank J. Craparo that it would be wise to stick with a more traditional approach of removing cerclage in a woman who has preterm premature rupture of membranes ("Cerclage in PPROM May Be Safe if No Infection," May 1, 2002, p. 9).
Dr. Lynn Montgomery's argument that there is theoretical benefit but no documented benefit to leaving the stitch in is nonsensical.
Cerclage itself has theoretical benefit but has been proven to be minimally efficacious.
The balance between the theoretical advantage of leaving the stitch in place versus the documented occurrence of increased risks for infection and cervical lacerations from retained suture with labor is clearly tilted toward making recommendations to our patients to have the suture removed.
