SAN DIEGO -- Small-bowel obstruction after abdominal surgery can be expectantly managed using nasogastric decompression, Dr. Sharif H. Ellozy said at the annual meeting of the American Society of Colon and Rectal Surgeons.
Within 6 days, 22 of 23 cases managed expectantly resolved. The remaining case did not respond. "Exploration at this point seems warranted even in the absence of clinical deterioration," said Dr. Ellozy of Mount Sinai School of Medicine, New York.
He presented a prospective study of 242 consecutive patients followed for 30 days after abdominal surgery. There were 23 cases (9.5%) of early postsurgical small-bowel obstruction as defined by initial return of bowel function followed by crampy abdominal pain, vomiting, and radiographic evidence of mechanical intestinal obstruction.
