UNIVERSAL CITY, CALIF. -- A number of patients can fall through the cracks when it comes to ordering thyroid testing in early pregnancy, so Dr. Jorge H. Mestman likes to set a low threshold for screening, he said at a meeting of the Obstetrical and Gynecological Assembly of Southern California.
"In our institution, 20% of patients with hypothyroidism in pregnancy have been diagnosed for the first time in pregnancy," even though they may have been hypothyroid previously but were unaware of the condition, said the professor of medicine, obstetrics, and gynecology at the University of Southern California, Los Angeles.
Dr. Mestman recommends ordering thyroid tests for pregnant patients with any of the following conditions:
* Presence of a goiter, which is never normal in pregnancy.
* Personal or family history of thyroid disease.
* Type 1 diabetes mellitus.
* Any other autoimmune disease.
