When used as a carrier, handle could break and allow infant to tumble to ground.
Products: Approximately 4 million rear-facing safety seats/ carriers made 1/91-7/97 and sold for between $40 and $130. Suspect products bear the following model designations: 11-564; 11-565; 11-570; 11-575; 11-576; 11-578; 11-595; 11-597; 11-598; 11-599; 11-600; 12-565; 4513; 4515; 4516; 4518; 4551; 4557; 4558; 4559; 4564; 4565; 4566; 4568; 4569; 4575; 4576; 4589; 4591; 4593; 4594; 4595; 4597. Date of manufacture--010191 through 073197 and model number appear on label on side of seat. Device has a molded, one-piece plastic handle that's either white, gray, or tan. Century infant seats covered in the January 1999 CONSUMER REPORTS were made after the dates in question and are not subject to recall.
What to do: Call Century at 800 865-1419 (or visit www .centuryproducts.com) for free replacement handle. You'll need to have the seat in front of you when contacting the company. Consumers should not carry the seat by handle until handle has been replaced, but they can continue to use the product as a safety seat.
* Cosco Options 5 high chair
Seat could collapse and allow child to tumble to ground. Some chairs have faulty metal restraint anchors that may allow child to fall out.
Products: 1 million high chairs, model number 03-286, made 12/1/97-8/11/00 and sold at mass-merchandise, juvenile- products, and major discount department stores for $40 to $50. Model number and date of manufacture appear on bottom of seat. Product can be used as high chair, infant feeding chair, youth chair, play chair, and booster seat. Seat has four tray positions, vinyl seat pad, and removable footrest. "Cosco" appears on leg support bar, on tray, and on sticker on bottom of seat. Restraint system consists of waist/crotch belt and plastic T-bar attached to tray. The Consumer Product Safety Commission and Cosco have received 168 reports of incidents involving the seats or restraints, including 57 injury reports. Collapsing-seat hazard exists with seat in either upright or reclining position.
