ORLANDO, FLA. - Botulinum toxin A, when injected into the external sphincter or bladder walls, allows some chronically incontinent patients to regain bladder control for up to 6 months, Dr. Michael Chancellor reported at the annual meeting of the American Urological Association.

The injections are useful for patients who have voiding dysfunctions that stem from sphincter spasticity, including those with multiple sclerosis, spinal cord injury, stroke, and overactive bladder and interstitial cystitis, said Dr. Chancellor, a urology surgeon in Pittsburgh.

Botulinum toxin A (Botox) can also be injected into the bladder wall to help alleviate some of the pain associated with bladder spasms, he said.

"This is nature's most perfect poison, but it is also a robust drug that has applications far beyond the cosmetic use for which it is known," Dr. Chancellor said in a press conference describing his 3-year experience in treating lower urinary tract dysfunctions with the neurotoxin.

Dr. Chancellor administered a total 100-200 units of botulinum A toxin (Botox) to each of 53 patients with sphincter spasticity. The toxin was divided into four equal doses and injected in the sphincter's four quadrants. For 22 patients with overactive bladder, he administered a total of 100-300 units diluted in 4-30 mL sterile saline, injected into 20-40 sites in the bladder walls. ...
