The US Federal Aviation Administration (FAA) is investigating a near miss incident that occurred at Washington Reagan National Airport on 14 May.

According to a report in The Washington Post on 25 June, the FAA has already indicated in a preliminary report that an air traffic controller may have been at fault. The controller involved apparently cleared a Piper PA-27 aircraft to land on one runway, then also erroneously cleared a US Airways Boeing 737 aircraft to take off from an intersecting runway.
