The dread of "being kept alive" is a preoccupation of many elderly Americans "Let me die, I don't want to be a burden to my family," and "I want to have a natural death. No machines ! " are pleas often heard from residents of nursing homes.

The increasing concern of patients of all age groups to remain autonomous when they are dying demands that the health care team obtain prior directives. Only when the competent patient's wishes have been recorded can the caretakers have any confidence that they are acting in the patient's best interest.
