The anticonvulsant agent valproic acid (Depakene, Depakote) is increasingly used for management of other medical and psychiatric conditions such as manic episodes associated with bipolar disorder, neuropathic pain, essential tremor, and diabetic neuropathy, as well as in prophylaxis of migraine. (1) The drug is generally well tolerated with adverse effects that are mostly mild and usually reversible. The following case describes an elderly patient who developed myopathy as a result of valproic acid therapy for management of her mania. (2)
