Camp ALA Registration Form

June 23-29, 2005--Annual Conference and Exhibition, Chicago, IL

Camp ALA is open to children aged 6 months--17 years from 7:30 am to 6:00 pm Friday, June 24-Tuesday, June 28. The total cost for the program is $80 per child per day. Parents pay $48/child/day, and ALA subsidizes $32/child/day. A $10 per child nonrefundable registration fee is also required. The per day price includes field trips on Saturday, Sunday and Monday for children aged 6-17 years. Field trip topics to be announced. The camp fee and field trips do not include lunch. Parents can purchase lunch on this registration form or bring lunch. The deadline for registration is May 6, 2005. Although every effort will be made to accommodate registration after the deadline and on-site, ACCENT cannot guarantee acceptance of children unless they are pre-registered.
