LAS VEGAS -- Findings from ultrasound and power flow Doppler imaging of uterine leiomyomas can differentiate suspected leiomyosarcomas that need further work-up from leiomyomas that require surgical or conservative management, Dr. Annalisa Amadio said.
A study comparing imaging and histologic findings from 257 uterine myomas identified large size and increased vascularity as the main characteristics that should increase suspicion of leiomyosarcoma, she said at the annual meeting of the American Association of Gynecologic Laparoscopists.
All patients underwent ultrasonographic and color flow Doppler imaging to evaluate myomas followed by myomectomy or hysterectomy.
Histologic diagnoses included 8 leiomyosarcomas, 21 cellular leiomyomas, 3 myomas of uncertain malignant potential, and 225 benign leiomyomas.
All single myomas larger than 8 cm in diameter on ultrasound were leiomyosarcomas. All the leiomyosarcomas showed marked peripheral vascularization on power flow Doppler, compared with 53% of other myomas.
