I was confused by the article "Careful Monitoring Called Key to Tocolysis" (Dec. 15, 2003, p. 13).
Since Dr. Washington C. Hill discussed 20-36 weeks' gestation as a time period to use tocolytics, he appears to advocate the use of tocolytics and steroids before 23 weeks and after 34 weeks. If steroids have no benefit before 23 weeks or after 34 weeks, and we use tocolytics to provide enough time for steroids to get on board, as Dr. Hill suggested, even though the available tocolytics are associated with adverse effects for mother and fetus, why would a physician use tocolytics before 23 weeks or after 34 weeks?
