Abstract
The objective of this study is to determine whether telehome monitoring of patients at high risk of hospital readmission will improve accessibility, quality and efficiency of health care. Patients with heart failure and angina at high risk of hospital readmission were telehome monitored for three months after hospital discharge. Patients were linked to the University of Ottawa Heart Institute through a home monitoring station to which they regularly transmitted their weight, vital signs, and electrocardiograms.
